1. 普通程序受理条件(来源:住宅专项维修资金使用审核标准(京建发【2010】)) (1)提交材料齐全; (2)核验所提交材料复印件与原件是否一致,基本信息与提供证照证件是否一致,提交材料是否按要求加盖单位公章; (3)申请表上收款单位,开户银行及收款帐号应准确,收款单位开户银行应为本市商业银行; (4)工程预算书是否由具有工程造价执业资格的造价师或造价员签字盖章; (5)施工企业资质是否与所申请维修工程内容一致; (6)分摊明细表中业主签字栏,是否达到由资金列支范围内专有部分占建筑物总面积三分之二以上的业主且占总人数三分之二以上的业主签字同意; (7)专业部门出具的鉴定报告或行业主管部门出具的整改意见书是否在有效期限内; (8)所申请维修工程是否在《住宅专项维修资金管理办法》(建设部、财政部令第165号)、《北京市住宅专项维修资金管理办法》(京建物[2009]836号)、《关于印发<北京市住宅专项维修资金使用审核标准>的通知》(京建发[2010]272号)规定的列支范围内; (9)核对专业部门出具的鉴定报告或行业主管部门出具的整改意见书是否与所申请维修工程一致; (10)核对工程总造价(工程预算)及分摊金额是否准确。 2.应急程序受理条件 (来源:住宅专项维修资金使用审核标准(京建发【2010】)) 发生下列危及房屋使用安全的紧急情况,需要立即对共用部分进行维修、更新、改造的,按照有关规定使用住宅专项维修资金: (1)屋面防水损坏造成渗漏的; (2)电梯故障危及人身安全的; (3)高层住宅水泵损坏导致供水中断的; (4)楼体单侧外立面五分之一以上有脱落危险的; (5)专用排水设施因坍塌、堵塞、爆裂等造成功能障碍,危及人身财产安全的; (6)消防系统出现功能障碍,消防管理部门要求对消防设施设备维修、更新、改造的; 3.公有住房售房单位、物业服务企业或相关业主申请维修的房屋需为申请区行政管辖范围内的房屋; 4.提交材料齐全; 5.核验所提交材料复印件与原件是否一致,基本信息与提供证照证件是否一致,提交材料是否按要求加盖单位公章; 6.申请表上收款单位,开户银行及收款帐号应准确,收款单位开户银行应为本市商业银行; 7.工程预算书是否由具有工程造价执业资格的造价师或造价员签字盖章; 8.施工企业资质是否与所申请维修工程内容一致; 9.所申请维修工程是否在《住宅专项维修资金管理办法》(建设部、财政部令第165号)、《北京市住宅专项维修资金管理办法》(京建物[2009]836号)、《关于印发<北京市住宅专项维修资金使用审核标准>的通知》(京建发[2010]272号)规定六种紧急情况; 10.核对工程总造价(工程预算)及分摊金额是否准确。 11.申请人需为 (1)小区的业主委员会; (2)小区的物业公司; (3)小区所在地的居委会或街道办事处。 12. 提交的证照证件或批复文件齐全完整,如提交复印件,应完整清晰并与原件一致; 13. 申请材料所载基本信息与提交的证照证件或批复文件所载内容一致; 14. 需使用A4纸打印或复印,需按照申请材料顺序装订成册; 15. 材料真实有效,无虚假信息。

受理条件 1. 普通程序受理条件(来源:住宅专项维修资金使用审核标准(京建发【2010】)) (1)提交材料齐全; (2)核验所提交材料复印件与原件是否一致,基本信息与提供证照证件是否一致,提交材料是否按要求加盖单位公章; (3)申请表上收款单位,开户银行及收款帐号应准确,收款单位开户银行应为本市商业银行; (4)工程预算书是否由具有工程造价执业资格的造价师或造价员签字盖章; (5)施工企业资质是否与所申请维修工程内容一致; (6)分摊明细表中业主签字栏,是否达到由资金列支范围内专有部分占建筑物总面积三分之二以上的业主且占总人数三分之二以上的业主签字同意; (7)专业部门出具的鉴定报告或行业主管部门出具的整改意见书是否在有效期限内; (8)所申请维修工程是否在《住宅专项维修资金管理办法》(建设部、财政部令第165号)、《北京市住宅专项维修资金管理办法》(京建物[2009]836号)、《关于印发<北京市住宅专项维修资金使用审核标准>的通知》(京建发[2010]272号)规定的列支范围内; (9)核对专业部门出具的鉴定报告或行业主管部门出具的整改意见书是否与所申请维修工程一致; (10)核对工程总造价(工程预算)及分摊金额是否准确。 2.应急程序受理条件 (来源:住宅专项维修资金使用审核标准(京建发【2010】)) 发生下列危及房屋使用安全的紧急情况,需要立即对共用部分进行维修、更新、改造的,按照有关规定使用住宅专项维修资金: (1)屋面防水损坏造成渗漏的; (2)电梯故障危及人身安全的; (3)高层住宅水泵损坏导致供水中断的; (4)楼体单侧外立面五分之一以上有脱落危险的; (5)专用排水设施因坍塌、堵塞、爆裂等造成功能障碍,危及人身财产安全的; (6)消防系统出现功能障碍,消防管理部门要求对消防设施设备维修、更新、改造的; 3.公有住房售房单位、物业服务企业或相关业主申请维修的房屋需为申请区行政管辖范围内的房屋; 4.提交材料齐全; 5.核验所提交材料复印件与原件是否一致,基本信息与提供证照证件是否一致,提交材料是否按要求加盖单位公章; 6.申请表上收款单位,开户银行及收款帐号应准确,收款单位开户银行应为本市商业银行; 7.工程预算书是否由具有工程造价执业资格的造价师或造价员签字盖章; 8.施工企业资质是否与所申请维修工程内容一致; 9.所申请维修工程是否在《住宅专项维修资金管理办法》(建设部、财政部令第165号)、《北京市住宅专项维修资金管理办法》(京建物[2009]836号)、《关于印发<北京市住宅专项维修资金使用审核标准>的通知》(京建发[2010]272号)规定六种紧急情况; 10.核对工程总造价(工程预算)及分摊金额是否准确。 11.申请人需为 (1)小区的业主委员会; (2)小区的物业公司; (3)小区所在地的居委会或街道办事处。 12. 提交的证照证件或批复文件齐全完整,如提交复印件,应完整清晰并与原件一致; 13. 申请材料所载基本信息与提交的证照证件或批复文件所载内容一致; 14. 需使用A4纸打印或复印,需按照申请材料顺序装订成册; 15. 材料真实有效,无虚假信息。
